The required Statement of Death must be completed before a death can be registered and permits issued. A funeral director will meet with your family at a comfortable location to gather information needed for the required forms and documents.

When you are able, it may help to gather the date and place of birth and death, marital status, occupation, Social Insurance Number, and the names and birthplaces of the deceased’s parents. Your funeral director will guide you through the required documents and signatures.
